The 36-year-old wicketkeeper on Friday woke up ill and has withdrawn from the four-day state game against Victoria at the MCG.
Paine was due to captain the Tigers in their must-win game, but will be forced to be tested for coronavirus instead.
Fast bowler Jackson Bird will lead Tasmania in Paine's absence.
The Tigers will bat first after Victoria captain Peter Handscomb won the toss and elected to bowl.
